HOUSTON – Information about more than 1,800 people may have been exposed after a box of documents was found next to a dumpster in The Heights, according to officials.
Officials at the Texas Health and Human Services Commission said a box containing client forms was found next to unsecured dumpster at the agency’s East 40th Street complex. They said the records were put beside the dumpster on April 21 and discovered a day later.
The forms contained personal information such as names, client numbers, dates of birth, case numbers and phone numbers, officials said. Some of the documents may have also contained mailing addresses, Social Security numbers, health information and bank account numbers.
A spokesperson told KPRC 2, “We are taking steps to remedy this incident. We are mailing out letters to each of the individuals who may have been affected by this breach and offering them free credit-monitoring services. We are also reviewing our processes and procedures for disposal of documents that contain clients’ personal information and working to prevent this type of event from occurring in the future.”
